Fruiting Body: cap 6-15 cm; stalk central, 7-15 cm long, 1-2 cm diameter. Spore Print Color: purplish brown. Spores: basidiospores 5-6.5 x 3-4.5 µm, ellipsoid, mooth, dark brown.

Diagnostic macroscopic feature: white mushroom that stains yellow with KOH, with free gills that are dark brown when mature, with white stalk and annulus growing on ground in woods or wooded yards
